The words of which the initial letter is capitalized have meanings defined under the following conditions. The following definitions shall have the same meaning regardless of whether they appear in singular or in plural.
For the purposes of this Cancellation and No-Show Policy:
Account means a unique account created for You to access our Service or parts of our Service.
Company (referred to as either "the Company", "We", "Us" or "Our" in this Policy) refers to PrimoMD, LLC, 3040 N Wickham Rd, Suite 6, Melbourne, FL 32935.
Service refers to the Website.
Website refers to PrimoMD, accessible from https://www.primomd.com
You means the individual accessing or using the Service, or the company, or other legal entity on behalf of which such individual is accessing or using the Service, as applicable.
If cancellation is necessary, Company requires that You call (321) 226-8607 at least 12-hours in advance. A cancellation is considered late when the appointment is cancelled less than 12-hours before the appointed time. A "no-show" is when You miss an appointment without cancelling. In either case, Company will charge You a $50 missed appointment fee. Note that arriving later than 20-minutes to appointed time is considered a "no-show" as indicated above.
COMPANY RESERVES THE RIGHT TO EDIT, OR REMOVE ANY PRODUCT, APPOINTMENT OR ACCOUNT FROM OUR SERVICE FOR ANY REASON OR NO REASON AT ALL. COMPANY RESERVES THE RIGHT TO CANCEL ANY ORDER OR ANY APPOINTMENT FOR ANY REASON INCLUDING BUT NOT LIMITED TO PRICING ERRORS, DOCTOR AVAILABILITY OR QUANTITY ISSUES.
